Mikami is a tall man, standing at around 6'0", with a thin but still athletic build. His skin is somewhat pale but not overly so. His hair is black and messy, but manages to frame his face in such a way to make him look elegant. (This is the result of careful styling.) His eyes are dark brown, nearly black, and usually kept very narrow. While at work he wears the standard-issue white coat; he generally tends to wear dark green or black suits under other circumstances. He wears black-framed glasses, but occasionally takes them off while relaxing. His posture is always straight and upright.

Mikami is a serious, studious young man, who is meticulous and perfect in everything he does, to the point of obsessive-compulsive behavior. His life is fitted to a tight schedule, and any deviation from it can ruin his entire day - he'll spend it anxious and worrying, unable to focus on his work. He likes to view things as neatly fitting into categories, especially people - he views others as either being 'good' or 'evil' with no room for grey between those two extremes. He isn't a sociable person, and spends most of his time on his own, away from the seemingly imperfect and chaotic world. However, he's not a (deliberately) cruel man - while he can be strict with them, he really does want his patients at the mental hospital to recover, and does what he can to facilitate their redemption. After all, his real goal is to save good people from evil forces, not simply to punish evil. However, if he declares someone 'unredeemable', he will do everything in his power to make sure they never see the light of day again, in order to keep them from harming others. He's also strongly religious, and certain that the punishment of evil is part of God's will.

When he was a child Mikami was at the top of every subject and respected among his peers apart from a selected few who thought humiliation and bulling was some sort of affection. Mikami knew the risks of defending against those peers and even so he leaped in and fought them off no matter the amount of people that objected to Mikami’s large craving of justice. Yet Mikami was still beaten to the ground, pulled and pushed, shoved and shamed with clothes torn off and bruises that merged into his skin and as he got older, it was harder to keep up his righteous acts of kindness.

Some time later Mikami received some shocking news hearing that his mother, along with a few bullies, got into a car accident and all of them happened to die. Mikami was frightened and yet delighted when he heard the news while still continuing his act of justice.

One day when Mikami grew up and got a job as a lawyer, he looked at the newspaper and those eyes of his widened.

Kira was his new god, the god of justice, who will cleanse the world of chaos and start a new one of order, the thought fluttered in Mikami’s heart and appeared on every event to celebrate the new god to only to be noticed and he was, in which his obsession with Kira grew even larger.

Kira, or Light, chose Mikami to carry out his judgments while he was caught up in some problems and Mikami was overwhelmed by this but he took the judgments a bit to far, eliminating those who had regretted what they did and not to mention useless people who didn’t deserve to be in the world, he also picked a new host for Kira who turned out to be Light’s college girlfriend.

It was unfortunate that Mikami screwed up in the end, he killed the anchorwoman but was also oblivious to the man that followed him, the man got a hold of Mikami’s note book and that lead to Kira’s downfall. Kira was led to the yellow box, a simple warehouse where a showdown between Kira and the successor of L took place. Light instructed Mikami to come as well but the notebook he had in his hands was false.

Mikami was hurt, very much as he heard the words come flowing from Kira’s mouth saying that Mikami was unknown to him and Mikami just snapped, he convinced himself that Light was not Kira but only a child with a weapon of mass murder, these thoughts soon after made Mikami insane and watched his god die infront of him before being taken away to a mental hospital and notified as insane.

Mikami's childhood was much the same - during his early life in Elysium he watched the unrighteous abuse those who were just and hated it, and after suffering far too much abuse the people Mikami had rejected most (his mother and the bullies) were all judged. The primary difference in his early childhood was that he knew they would be judged - floating above each person's head was a name and a number slowly counting down. He noticed that the unrighteous tended to have much smaller numbers, which pleased him deeply. The shortness of the number floating above his mother's head frightened him up until the moment she betrayed him by telling him to stop his crusade of justice. Then he knew that just like the others, she was meant to be judged. Her death came as no real surprise.

He decided that the power he had been given was a gift from God - allowing him to be confident that the unrighteous would be judged. When he saw a good person with a short number, he comforted himself by assuring himself that the person had to be hiding some dark secret. This strengthened his belief in God's justice deeply.

He became a psychologist for violent and criminal patients in order to help rehabilitate those who were considered "evil" so that they could possibly partially redeem themselves and do good for society before their judgment. When someone proved irredeemable, he simply locked him away so that they could await their elimination in a place where they could do no harm to others. A few of them vanished into the Alpha Omega Research Facility, but Mikami cared little.

Then he met his newest patient: Beyond Birthday, AKA B. A dangerous serial killer and madman. And it was Mikami's job to somehow redeem him.

The attempts were less than successful, and so Mikami put B on more and more medication in order to try to fix him. Nothing worked, and B's companions (notably Heine, one of Mikami's outpatients and a dangerous man in his own right) made multiple attempts to free B using blackmail and threats. Finally a direct threat forced Mikami to release B, although he managed to install guards around the man.

B ended up saving Mikami's life when yet another of Mikami's patients, Mello, threatened to take it. Mikami, while he by no means bonded with B, began to hate him somewhat less. And after much discussion and the realization that B would never be fully redeemed, Mikami struck a deal with the man.

There were many that needed judgment that seemed to be coming too slowly. B would simply speed up the process by focusing his destructive impulses in a new direction. Mikami removed the guards on B and provided lists of names and crimes, and B struck as his force of justice. There was no way to link it to either of them.

It was the perfect arrangement for both of them.
